چکیده

physiology of the pancreas graft, which is heterotopic and denervated, together with the immunosuppressive treatment and relatively impaired renal function might be responsible for minor differences in hormonal and metabolic patterns. Results comparable to ours have been described in non uraemic type I diabetic patients treated with artificial endocrine pancreas; continuous subcutaneous, intravenous, or intraperitoneal insulin infusions; and intensified conventional insulin treatments.31-35 Pancreas transplantation seems an ethical proposition in type I diabetic patients undergoing kidney transplantation for chronic renal failure and in whom immunosuppression is to be used. In these cases pancreas transplantation, when successful, leads to the restoration of normal metabolic homoeostasis without hormone replacement.

Accidental and Surreptitious Intake of Oral Anticoagulants Including Older and Newer Agents - A Diagnostic Dilemma

The newer oral anticoagulants including dabigatran, rivaraxoban, apixaban and edoxaban are replacing warfarin in the management of atrial fibrillation and venous thromboembolism. Accidental and surreptitious overdose of warfarin has been reported, but so far there is no reported case of overdose involving NOACs. Overdose of warfarin prolongs coagulation tests including PT and APTT. This gives a...

Metabolism of meso-2,3-dimercaptosuccinic acid in lead-poisoned children and normal adults.

Meso-2,3-dimercaptosuccinic acid (DMSA, or succimer) is an oral chelating agent for heavy-metal poisoning. While studying the urinary elimination of unaltered DMSA, altered DMSA (i.e., its mixed disulfides), and lead in children with lead poisoning, we observed a pattern of urinary drug elimination after meals suggestive of enterohepatic circulation. The excretion of lead in urine patterned the...

Gastrointestinal Elimination of Perfluorinated Compounds Using Cholestyramine and Chlorella pyrenoidosa

Background. While perfluorinated compounds (PFCs) are a family of commonly used synthetic compounds with many applications, some PFCs remain persistent within the human body due, in part, to enterohepatic recirculation and renal tubular reabsorption. With increasing recognition of potential harm to human health associated with PFC bioaccumulation, interventions to facilitate elimination of thes...

Warfarin Re-initiation Gone Awry: A Case of Inadvertent Overdose Mandating Critical INR Management.

Warfarin is a commonly used anticoagulant for the primary or secondary prevention of thrombotic events.1 Tremendous inter-individual variability in warfarin dosing is apparent, necessitating frequent monitoring via the international normalized ratio (INR). The most feared complication of warfarin therapy is major bleeding, and the risk of this problem increases when the INR exceeds 4.0.2 Critic...

Effect of Oral Anticoagulant Therapy on Coagulation Activity and Inflammatory Markers in Patients with Atrial Fibrillation Undergoing Ablation: A Randomized Comparison between Dabigatran and Warfarin

Atrial fibrillation (AF) is associated with inflammatory and hypercoagulability state. Previous studies evaluated the safety and efficacy of dabigatran and warfarin in prevention of thrombothic complications. This study was intended to assess the influence of these drugs on hemostatic and inflammatory markers among patient underwent pulmonary vein ablation.   A total of 100 patients with AF w...
